Physical Therapist Job Requirements

• Minimum of 1 year of experience as a Physical Therapist required
• Current Physical Therapist license in the applicable state
• Experience in homecare, home health, or community-based patient care preferred
• Strong assessment, treatment planning, and patient education skills
• Ability to work independently and manage a mobile patient caseload
• Excellent communication, documentation, and care coordination skills

Responsibilities

• Evaluate patients in the home setting and develop individualized physical therapy treatment plans
• Provide therapeutic interventions to improve strength, mobility, balance, and functional independence
• Educate patients and caregivers on exercises, safety strategies, and ongoing care plans
• Document patient progress, treatment outcomes, and care coordination activities accurately and timely
• Collaborate with interdisciplinary team members to support continuity of care
• Adjust treatment plans based on patient response and evolving clinical needs
