Job Details

• Position: Certified Nursing Assistant (CNA)
• Setting: Post-Acute Skilled Nursing and Rehabilitation
• Location: Middlebury, VT 05753
• Assignment Length: 16 weeks
• Schedule: 12-hour day shifts
• Hours Per Week: 36
• Weekly Estimated Pay: $1,220 - $1,310
• Float Requirement: Yes, with assignment across Post-Acute, Long-Term Care (LTC), and Memory Care as needed
• Weekend/Holiday Expectations: Minimum every other weekend; ability to work holidays is required
• Technology: EPIC experience within the past 2 years is required

Job Requirements

• Active Certified Nursing Assistant certification
• At least 2 years of CNA experience required
• Prior travel CNA experience required
• Experience in long-term care, skilled nursing, or post-acute care preferred
• AHA BLS certification required
• Recent EPIC experience within the last 2 years required
• Ability to work a flexible schedule, including holidays and rotating weekends
• Strong communication skills and a patient-centered approach to care
• Reliable transportation is strongly recommended due to limited public transit in the Middlebury, VT area

Responsibilities

• Provide compassionate hands-on care to residents and patients in a post-acute rehabilitation setting
• Assist with activities of daily living (ADLs), comfort measures, and routine patient support
• Support the care team with patient transports, phlebotomy support, EKGs, nebulizer-to-inhaler support, and starting IVs as assigned
• Float between Post-Acute, LTC, and Memory Care units based on patient care needs
• Maintain accurate documentation in EPIC and follow established facility procedures
• Work collaboratively with nursing and ancillary team members to promote safe, high-quality patient care
• Adapt to changing assignments while helping ensure smooth daily operations across the unit
