Job Requirements

• Active Occupational Therapist license
• Minimum 1 year of Occupational Therapy experience required
• Home health experience strongly preferred
• Experience with HCHB documentation preferred
• Bachelor's degree required
• Strong communication, organization, and time-management skills
• Ability to provide patient-centered care in a home-based setting

Responsibilities

• Evaluate and treat patients in their homes as part of a home health caseload
• Develop and implement individualized occupational therapy treatment plans
• Support patients in improving functional independence and daily living skills
• Document visits accurately and maintain timely clinical records
• Collaborate with the care team to ensure quality, coordinated patient care
• Travel within the Fort Worth coverage area while meeting productivity expectations
