Job Requirements

• Active North Carolina Physical Therapy Assistant (PTA) license
• At least 1 year of PTA experience preferred
• Home health experience required
• Strong communication, organization, and collaboration skills
• Ability to work independently in a patient home setting
• HCHB experience is a plus

Responsibilities

• Provide skilled physical therapy interventions in patient homes under the supervision of a physical therapist
• Support patients in achieving mobility, strength, balance, and functional goals
• Deliver home health care across the designated coverage area in a timely and professional manner
• Document visits accurately and maintain clear communication with the care team
• Follow established care plans and adjust treatment support based on patient needs
• Represent the care team with professionalism, compassion, and a patient-first attitude
