• Weekly Estimated Pay: $1,742 - $1,935
• Location: Baltimore, Maryland

• Assignment Length: 14-week travel assignment
• Schedule: 5 shifts per week
• Shift Duration: 8 Hour Days
• Hours: 40 hours per week
• Weekend Requirement: 2 weekends per month
• On Call: None
• Charting Systems: Cerner and AllScripts
• Dress Code: Business casual or any color scrubs

Job Requirements

• 3 years of social work experience required
• Maryland LMSW or LCSW required
• Master of Social Work (MSW) required
• Recent Emergency Department social work experience preferred
• Teaching hospital experience preferred
• Short-term acute care inpatient Medical Social Worker experience required
• Experience with discharge planning, care coordination, and multidisciplinary collaboration required
• Advance directive experience required
• Ability to float within scope based on facility needs

Responsibilities

• Complete initial social work evaluations and ongoing assessments of patient response to treatment
• Develop and update individualized plans of care in collaboration with the healthcare team
• Identify patient and caregiver needs and coordinate appropriate discharge resources
• Support safe transitions of care through effective discharge planning and case management
• Coordinate home care services, equipment, referrals, and post-acute placement needs, including rehabilitation or long-term care facilities
• Participate in care conferences and communicate recommendations clearly with the interdisciplinary team
• Support Emergency Department and Pediatric Emergency Room patient populations in a high-volume acute care environment
• Assist with advance directives and other psychosocial care planning needs
