• Schedule: Friday, Saturday, Sunday, and Monday; 8:00 AM to 6:00 PM
• Call: Limited emergency-only call may be scheduled
• Weekly Estimated Pay: $2,120-$2,230

Job Requirements

• Active Registered Nurse (RN) license in good standing
• At least 1 year of home health or hospice nursing experience
• OASIS experience required
• Experience traveling from patient to patient for home visits
• Ability to manage changing schedules and work independently in the field
• Valid driver's license in good standing
• Current BLS certification required
• Experience with HealthWyse (Careficient) or similar EMR preferred
• Comfortable with outpatient-style care delivery in the home setting

Responsibilities

• Provide high-quality nursing care to patients in their homes
• Complete skilled assessments, documentation, and care planning
• Deliver hospice and home health services with professionalism and compassion
• Use OASIS documentation accurately and efficiently
• Travel between patient locations while maintaining excellent time management
• Coordinate care and communicate effectively with the interdisciplinary team
• Adapt to schedule changes and respond to patient needs as they arise
• Support safe, patient-centered care in a community-based environment
