• Schedule: 8-hour day shifts, 8:00 AM to 4:30 PM
• Hours Per Week: 40
• Duration: 13 weeks
• Weekly Estimated Pay: $2,620 - $2,760
• Assignment Type: On-site patient care coordination role
• Call: No call required

Job Requirements

• Active California Registered Nurse license
• Current CPR certification
• At least 1 year of recent case management experience in an acute care setting
• Strong clinical assessment, communication, and care coordination skills
• Ability to work collaboratively with patients, families, physicians, and multidisciplinary care teams
• Comfort with utilization review, discharge planning, and transitions of care
• Experience supporting efficient care coordination in a hospital or acute care environment is highly valued

Responsibilities

• Coordinate care and services for assigned patients throughout an episode of illness or treatment
• Apply the nursing process, including assessment, planning, intervention, and evaluation
• Support appropriate utilization of resources and promote efficient, cost-effective care delivery
• Review admission and discharge criteria, observation status, readmissions, and related utilization cases
• Partner with the patient care team to help ensure timely, high-quality patient outcomes
• Serve as a resource for patients and families and help facilitate communication among providers
• Remain involved in the care process until the patient is discharged or transferred to the appropriate level of care
• Initiate clinical pathways based on admission diagnosis and support utilization management efforts
• May be assigned to support the Emergency Department as needed
