Job Requirements

• Active New York LPN license required
• Current BLS certification from the American Heart Association required

• Ability to provide care with professionalism, dignity, and respect

Responsibilities

• Provide direct patient care to residents in a long-term care and geriatric care setting
• Administer medications and support ongoing resident treatment plans
• Deliver wound care and monitor residents for changes in condition
• Collaborate with CNAs and other healthcare team members to support daily resident care needs
• Maintain accurate documentation and communicate effectively with the care team
