Job Requirements

• Minimum 3 years of inpatient CDI experience in a hospital or acute care setting
• Strong knowledge of ICD-10 coding guidelines, CMS regulations, and DRG reimbursement methodologies
• Experience with concurrent inpatient chart review and clinical documentation integrity practices
• Ability to identify principal and secondary diagnoses, including CC/MCC capture
• Understanding of Severity of Illness (SOI) and Risk of Mortality (ROM) review principles
• Excellent written communication skills for compliant provider queries and physician engagement
• Ability to work independently and maintain productivity in a structured on-site healthcare environment
• CCDS or CDIP certification preferred

Responsibilities

• Perform initial and follow-up concurrent reviews of targeted inpatient medical records
• Complete initial patient reviews within 24-48 hours of admission and subsequent reviews every 24-48 hours as appropriate
• Review an average of 18-25 patient charts per day while maintaining quality and productivity standards
• Identify and document conditions being evaluated, monitored, or treated during the inpatient stay
• Assign and validate principal and secondary diagnoses, including identification of CC/MCC opportunities
• Evaluate documentation for severity of illness, risk of mortality, and overall clinical accuracy
• Escalate cases involving patient safety and hospital-acquired condition indicators as appropriate
• Issue clear, compliant written queries when documentation is incomplete, conflicting, or ambiguous
• Collaborate with physicians, providers, and care teams to support accurate clinical documentation
• Apply official coding guidelines, ethical standards, and CDI documentation policies in daily review activities
• Use designated documentation and reporting systems for CDI review, data entry, and query tracking
