Overview

This is a full-time travel Physical Therapist position supporting home health services in a community-focused setting. You’ll work closely with patients and the care team to deliver high-quality rehabilitation therapy, complete evaluations, and guide patients through meaningful progress in their recovery journey.

Job Requirements

• Bachelor’s degree required
• Active Massachusetts Physical Therapist license
• At least 1 year of Physical Therapist experience required
• Home health experience strongly preferred
• Professional certification required
• HCHB familiarity is a plus
• Strong communication, organization, and time-management skills
• Ability to work independently while supporting collaborative patient care

Responsibilities

• Perform start-of-care assessments, evaluations, and skilled therapy visits
• Develop and implement patient-centered treatment plans
• Provide education and support to help patients improve function and mobility
• Coordinate care with the team and provide handoff/supervision to PTA as appropriate
• Document visits accurately and in a timely manner
• Monitor progress and adjust treatment plans based on patient needs
• Maintain productivity goals while delivering high-quality care
• Support safe, effective rehabilitation in the home setting
