Job Requirements

• Active Licensed Practical Nurse (LPN) license
• At least 1 year of Long Term Care or skilled nursing experience preferred
• Strong clinical assessment, medication administration, and resident care skills
• Ability to work rotating shifts and adapt to changing patient needs
• Experience with charting, documentation, and interdisciplinary communication
• Proof of flu vaccination or an approved medical or religious exception prior to start
• Ability to arrive the Thursday before the start date for employee health clearance

Responsibilities

• Perform resident assessments and provide ongoing nursing care in a Long Term Care setting
• Administer medications and treatments according to care plans and facility protocols
• Notify the physician or nurse practitioner of changes in resident condition
• Assist with admissions, discharges, and activities of daily living (ADLs)
• Provide care for gastrostomy tubes, urinary catheters, nebulized treatments, wound vacs, and CPAP support as assigned
• Promote resident comfort, safety, dignity, and quality of life
• Complete accurate charting and collaborate closely with the healthcare team
