• Schedule: 8:00 AM to 4:30 PM
• Hours Per Week: 40
• Guaranteed Hours: 36
• Estimated Weekly Pay: $2,030 - $2,190
• Patient Coverage: Ashtabula, Lake, and surrounding counties
• Productivity Expectation: 30
• Mileage: Reimbursed between patient visits

Job Requirements

• Active Physical Therapist license in Ohio
• Doctoral degree required
• At least 1 year of experience in home health, outpatient rehab, or a related physical therapy setting
• Strong case management skills and the ability to work independently in the field
• HCHB experience preferred
• Excellent communication, organization, and time-management skills
• Ability to travel between patient visits within the assigned service area

Responsibilities

• Provide skilled physical therapy services to patients in the home health setting
• Develop and implement individualized treatment plans based on patient needs, goals, and functional outcomes
• Perform assessments, progress evaluations, and timely clinical documentation
• Serve as a care coordinator and case manager for assigned patients
• Collaborate with the interdisciplinary care team to support safe, effective patient outcomes
• Educate patients and caregivers on mobility, therapy exercises, fall prevention, and safe home techniques
• Travel throughout the assigned Northeast Ohio coverage area to provide consistent patient care
