• Education: Master’s degree required

Job Requirements

• Active Washington Occupational Therapist license
• Master’s degree in Occupational Therapy or related field
• At least 1 year of experience in occupational therapy or home health preferred
• Experience with HCHB Home Health strongly preferred
• Strong clinical judgment and ability to work independently in the home health setting
• Professional certification required
• Business casual dress code

Responsibilities

• Complete patient care assessments in the home health setting
• Perform recertifications and update care plans as needed
• Conduct discharge assessments and coordinate safe transitions of care
• Deliver high-quality occupational therapy services tailored to each patient’s goals
• Document care accurately and efficiently in the required home health system
• Collaborate with the interdisciplinary care team to support positive patient outcomes
• Travel within the assigned Olympia, WA coverage area to provide visits across approximately 50 miles
