• End Date: 01/03/2027
• Duration: 13 weeks
• Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with on-call coverage and weekends as needed
• Shift Duration: 8 hours
• Hours Per Week: 40
• Weekly Estimated Pay: $1,830 - $1,930
• Setting: Home health and hospice
• Productivity Goal: 30 points per week
• Mileage Reimbursement: Provided for eligible travel

Job Requirements

• Active West Virginia RN license
• Minimum of 1 year of nursing experience required
• Home health experience strongly preferred
• Experience with HCHB and OASIS documentation preferred
• Strong assessment, documentation, and care coordination skills
• Ability to work independently and manage a varied patient caseload
• Comfortable with on-call responsibilities and weekend coverage

Responsibilities

• Assess, plan, implement, and evaluate nursing care for patients in the community
• Support health maintenance, disease prevention, and case management needs
• Collaborate with interdisciplinary team members to coordinate patient care
• Document patient status, progress notes, and required nursing records accurately
• Monitor medications and communicate changes in patient condition to the appropriate provider
• Coordinate patient appointments and follow-up care to support timely interventions
• Provide care plan oversight and assist with staff supervision as needed
• Ensure safe, compliant, and patient-centered care in all assignments
