Minimum Qualifications

• Master's degree in Social Work (MSW) required
• At least 1 year of medical social work experience in a healthcare or hospital setting
• Experience with discharge planning, care coordination, and working with Medi-Cal and underserved populations
• Ability to collaborate with RN case managers and multidisciplinary teams
• Strong communication, problem-solving, and organizational skills
• Compassionate, creative, and resourceful approach to complex psychosocial needs

Key Responsibilities

• Conduct comprehensive psychosocial assessments for patients with complex needs, including unhoused and Medi-Cal populations
• Develop and implement safe, creative discharge plans utilizing available community resources and care coordination
• Collaborate closely with RN case managers, physicians, and interdisciplinary teams to ensure continuity of care
• Advocate for patients and connect them with appropriate social services, housing supports, and community resources
• Navigate challenging situations with empathy and professionalism while documenting interventions accurately
• Contribute to a positive, team-oriented hospital environment and participate in quality improvement efforts
