Job Requirements

• Active Physical Therapist license in the state of Connecticut
• Doctoral degree required
• At least 1 year of Physical Therapy experience required
• Experience in home health, rehab, or outpatient therapy preferred
• Strong communication, organization, and clinical documentation skills
• Ability to work independently while contributing to a team-based care model
• Comfortable providing care in a home health setting
• Professional business casual attire or scrubs
