• Weekly Estimated Pay: $1,874 - $2,082
• Schedule: 5 shifts per week
• Shift Duration: 8 Hour Days

Job Requirements

• Acute care hospital inpatient case management experience required
• Previous travel nurse experience or Epic EMR experience required
• Active RN license required; Virginia RN license pending may be considered
• Strong care coordination, discharge planning, and interdisciplinary communication skills
• Ability to work effectively in a fast-paced inpatient hospital setting
• Current professional references and standard onboarding documentation required

Responsibilities

• Coordinate patient care across the continuum to support safe and timely discharge planning
• Collaborate with physicians, nurses, social workers, and interdisciplinary teams to develop effective care plans
• Review patient cases for medical necessity, level of care, and resource utilization
• Facilitate communication with patients and families regarding treatment plans and discharge needs
• Document case management activities accurately within Epic
• Support quality outcomes by identifying barriers to care and helping implement appropriate solutions
• Maintain compliance with hospital policies, regulatory standards, and case management best practices
