Travel Physical Therapist Job Details

• Location: Elizabethtown, Kentucky, with home health service coverage throughout Hardin, Meade, and Breckinridge counties
• Healthcare Setting: Home Health
• Weekly Estimated Pay: $2,030–$2,190
• Schedule: Monday through Friday, 8:00 a.m.–4:30 p.m.
• Shift Duration: 8-hour shifts
• Shifts per Week: 5
• Hours per Week: 40 guaranteed hours
• Contract Duration: 13 weeks
• Start Date: September 24, 2026
• End Date: December 23, 2026
• Weekend and On-Call Requirement: None regularly scheduled
• Travel: Local travel between patient visits is required; mileage between visits is reimbursed according to assignment policy

Travel Physical Therapist Job Requirements

• Doctor of Physical Therapy degree or equivalent doctoral-level physical therapy education
• Current, unrestricted Physical Therapist license in the state of Kentucky
• Current CPR certification
• At least 1 year of professional physical therapy experience; home health physical therapy experience is strongly preferred
• Current driver’s license, automobile insurance, and access to a dependable vehicle
• Ability to travel throughout the assigned service area and provide skilled physical therapy care in patients’ homes
• OASIS proficiency is preferred
• Strong communication skills, a positive attitude, and the ability to collaborate effectively with patients, families, caregivers, and the interdisciplinary healthcare team
• Professional healthcare attire and closed-toe shoes are required

Home Health Physical Therapist Responsibilities

• Evaluate patients’ functional status, mobility, endurance, pain, physical limitations, and therapy needs in the home setting.
• Develop individualized physical therapy plans of care in collaboration with the physician and interdisciplinary healthcare team.
• Provide skilled, evidence-based physical therapy interventions to relieve pain, restore function, improve mobility, and maintain the highest practical level of independence.
• Deliver home health physical therapy services within the scope of Kentucky physical therapy practice and according to the established plan of care.
• Monitor patient responses to treatment and promptly communicate changes in condition, progress, and concerns to the appropriate clinical team members.
• Educate patients, families, caregivers, and participating healthcare professionals regarding treatment plans, functional limitations, home exercise programs, and progress toward goals.
• Complete evaluations, progress notes, visit documentation, outcome reporting, and discharge summaries in accordance with applicable requirements and agency standards.
• Report evaluation findings, goals, projected frequency of care, and anticipated outcomes within required timelines.
• Coordinate care with nurses, physicians, physical therapist assistants, home health aides, and other members of the interdisciplinary care team.
• Provide appropriate supervision of physical therapist assistants and home health aides when permitted by state regulations and applicable policy.
• Participate in staff conferences, committees, in-service education, and ongoing professional development activities as requested.
• Maintain current clinical knowledge, licensure, certifications, and documentation practices consistent with professional standards of care.
