RN Case Manager Job Details

• Job Title: Registered Nurse Case Manager (Locum Tenens)
• Employment Type: Locum tenens (Travel nursing contract)
• Location: Glendale, CA 91204 – Greater Los Angeles Area
• Healthcare Setting: On-site at a respected hospital
• Start Date: September 29, 2025
• Contract Duration: 13 weeks
• Schedule: Monday-Friday, rotating weekends
• Hours per Week: 40
• Shift: 8 Hour Days
• Estimated Weekly Pay: $2,685 - $2,983

Registered Nurse Qualifications

• Active California RN license required
• Minimum 1 year of recent experience in case management, care coordination, or related nursing specialty
• Strong communication, organizational, and clinical assessment skills
• Ability to collaborate with interdisciplinary healthcare teams
• Experience in Same Day Surgery, Detox, Bone Marrow Transplant, Mental Health, or IDD is a plus

Case Management Responsibilities

• Coordinate patient care plans and facilitate seamless transitions of care
• Advocate for patients and ensure comprehensive healthcare needs are met throughout hospitalization
• Collaborate with physicians, social workers, and other clinical professionals to optimize patient outcomes
• Monitor patient progress, update care plans, and document case management activities
• Educate patients and families on available healthcare resources and discharge planning
