Licensed Practical Nurse Job Requirements

• Active Licensed Practical Nurse (LPN) license valid for practice in Colorado
• At least 1 year of practical nursing experience, preferably in long-term care, skilled nursing, medical-surgical, or special care settings
• Current Basic Life Support (BLS) certification
• Ability to work night shifts, variable scheduling, and required call coverage
• Strong communication, organization, clinical judgment, and teamwork skills

Long-Term Care LPN Responsibilities

• Assess residents and identify ongoing and changing healthcare needs
• Plan, implement, and coordinate individualized patient care
• Provide safe, compassionate nursing care to skilled nursing, extended-care, and special care unit residents
• Monitor patient conditions, document clinical findings, and communicate changes to appropriate care team members
• Collaborate with interdisciplinary team members to support quality outcomes and continuity of care
• Respond to patient needs during scheduled shifts and required call coverage
• Follow facility policies, clinical standards, and applicable Colorado nursing regulations
