Job Requirements

• Active California RN license required
• 3-5 years of experience in an acute care hospital setting required
• Case management, discharge planning, or utilization review experience strongly preferred
• Strong communication, collaboration, and care coordination skills
• Ability to work effectively in a fast-paced healthcare environment

Responsibilities

• Coordinate discharge planning for patients in the acute care setting
• Collaborate with interdisciplinary care teams to support safe and timely patient transitions
• Assess patient needs and identify appropriate post-acute resources and services
• Communicate with patients, families, and healthcare teams regarding discharge plans and barriers
• Document case management activities accurately within Cerner
• Support continuity of care and promote positive patient outcomes throughout the discharge process
